Affordable, agent-friendly dental coverage from Delta Dental — one of the largest dental networks in the nation — at a flat $25/month per person. Bundled with vision insurance for the same price. Available to U.S.-based Epique agents on the Dental Insurance page.

What's covered

  • Free Checkups and Cleanings.
  • Free Exams, X-Rays, Sealants and Space Maintainers.
  • Up to 80% off Root Canals and Oral Surgery.
  • Up to 80% off most other Basic Services.
  • Annual maximum: $750 per calendar year, renewing on the calendar reset.

Pricing

$25/month per person. The same $25 buys you both Dental and Vision — they're bundled. You can't enroll in one without the other; the cost is the same either way.

Why Delta Dental

  • Strong Network. One of the largest dentist networks in the country. Delta Dental contracts directly with their dentists (no leased networks), so the experience is consistent at any in-network office. More in-network dentists means lower out-of-pocket costs for you.
  • Dedicated Support. A dedicated account manager backs the plan. Acknowledgements within 24 hours, regular performance reports, and access to monthly wellness campaigns.

Sign up & access

Three buttons on the page:

  • Sign Up — opens the JotForm enrollment. Enrollment begins on the 1st of the following month. You'll get a confirmation email within 48 hours of submitting.
  • Log In — opens Delta Dental's member portal for already-enrolled agents.
  • Plan PDF — opens the official Delta Dental plan summary PDF.

Managing your plan

You don't get a mailed ID card — instead, ID cards are in the Delta Dental app and on the Delta Dental website, available for download and printing. The app is also where you check benefits, find a dentist, and look up your Enrollee ID.

To add dependants after initial enrollment, email [email protected]. New dependants are activated on the 1st of the following month.

Cancellation

To cancel, email [email protected] before the next billing cycle. Otherwise the benefit ends at the close of your current paid-up period. The plan is non-refundable.

If you can't find your Enrollee ID in the Delta Dental app or website, drop a note to the benefits team — they can pull it up for you.
