Your annual benefits
You get two $180 vouchers per year — that's $360 in annual eyewear credit. Each voucher can be used independently for any of:
- Prescription glasses (frames + lenses).
- Non-prescription glasses (e.g. blue-light computer glasses).
- Contact lenses.
Plus a free vision acuity exam with no additional cost.

Pricing & bundling
$25/month per person, bundled with dental insurance. The $25 covers both — you can't enroll in one without the other, and the cost is the same either way. See the Dental Insurance page for the dental side.
Sign up & access
- Sign Up — opens the JotForm enrollment.
- Login — opens member.myrevive.health, the Revive member portal where you'll see your vision card, find your details, and shop.
Once enrolled, you'll get an email from Revive within 24 hours. If you already have a Revive account, the Vision portal will be added to it automatically.
How it works
The benefit is virtual — designed for ordering through the portal rather than walking into a physical store. Order glasses or contacts through Revive's site, try them at home (or virtually), and your vouchers cover the cost up to $180 per voucher.
Family allowance
If you enroll yourself plus dependents:
- Each person gets their own annual benefit allowance.
- Allowances are not divided across the family — every dependent gets a full $360 in annual credit, just like you.
To cancel, email [email protected] before the next billing cycle. The plan is non-refundable, so cancelling mid-cycle ends coverage at the end of your paid-up period.
